我最近在crucial的網站訂購記憶體，但卻收到一封回信如下：


We have received your order #13964669, however it is our policy not to ship to an address we cannot verify by your issuing bank. In order to speed up your order you have the option of faxing or emailing me the top portion of your credit card statement. This will verify your name, billing address and account number.


請問：
1.crucial是要我信用卡帳單上的那些資料呢？
2.又這些資料再信用卡帳單上的那一部分呢？
